A rating of 5 is the best rating, and a rating of 1 is the worst.  Read each item carefully, and mark the number that best represents your feelings.  You do not need to sign your name.

1. PRESENTATION: The instructor’s presentations show
Preparation, organization and knowledge of subject taught.

2. INTEREST:  The instructor stimulated my interest in the subject.

3. CLASS TIME: The instructor uses class time effectively.

4. HOMEWORK: Work assigned by the instructor to be done outside of class was helpful. (Write N/A if no homework was assigned.)

5. HELPFUL: The instructor is helpful when difficulties arise, and is approachable in and outside of class.

6. COURSE OBJECTIVES: The instructor has met the course Objectives.

7. MATERIALS: The textbook(s) and handouts are relevant and used Effectively.

8. MOTIVATION: This course was interesting and useful.  It will help me to get what I want.
